What are eyelid lumps?

Eyelid lumps, including benign (chalazions, cyst of Moll, sebaceous cysts, Xanthelasma, papillomas =’warts’) and malignant tumours such as basal and squamous cell carcinomas are very common presentations in oculoplastic.

Through careful examination and thorough history to identify risk factors (such as fair skin, sun exposure, living in hot country, history of sunburns, taking immunosuppressant medication and sometimes syndromic such as Gorlin-Golz or basal nevus syndrome leading to multiple basal cell carcinomas from a young age.

Diagnosis is important as medical treatment with Hedgehog pathway inhibitors (such as Vismodegib) is available on NHS via skin cancer MDT and colleagues in oncology and NICE approved for these particular group of high risk younger patients.

In the face of a malignant tumour diagnosis around eye area such as basal cell carcinoma, I understand that it may initially feel overwhelming and scary, but I want to assure you that you are not alone on this journey.

Basal cell carcinoma is the most common form of skin cancer, and with early detection and expert care, the road to recovery can be positive.

Nodular basal cell carcinomas

Infiltrative basal cell carcinomas

I carry out the excision surgery and reconstruction (as a second stage if tissues do not come together after the excision). This is to ensure that the margins after excision are clear of tumour and significantly reduce the risk of recurrence.

The accepted clear margins differ depending on the type and aggressiveness of the tumour, which is establish during the histopathological (laboratory) analysis.

I discuss all of these after the report from the laboratory becomes available (it takes usually about 1 week).

If margins are close to tumour or not clear further excision will be needed sometime delaying the reconstruction.

I carry out all oculoplastic/eyelid reconstructions from simple ones such and direct closure/ rhomboid flaps to more complex such as Hughes flaps, advancement flaps, glabella flaps, skin grafts, cheek rotation flaps, forehead flaps.

Some of these require extensive dissection in the underlaying tissues and are best carried out under general anaesthetic or with local anaesthetic and sedation to maintain the comfort of the patient. Others are possible with local anaesthetic only.

Some flaps require closing the eye for 2-3 weeks and/ or a second stage intervention to divide the flap (for example Hughes, paramedian forehead flap). All options are discussed during the consultation.

The recurrences of previously excised malignant tumours are best managed through excision by MOHS surgery (carried out by my dermatology colleagues which ensure adequate and full clearance of the tumour.

After that stage I will do the reconstruction. Sometimes it is difficult to appreciate the size or depth of the defect until the MOHS surgery is done as this often ends up larger than originally thought. This can change the reconstructions plans.

Eyelids normally heal well (due to good and multiple blood supply) and you’ll be able to see the definitive results around 3-4 months postoperative, when swelling and bruising resolve and tissues heal.

Regular massage and application of an anti-scar gel silicone gel such as Dermatix can improve the final cosmetic result.

Skin remodellers containing polynucleotides such as Plinest or highly link hyaluronic acid such as Jalopro/ Prophilo complex can improve the result and scar quality, unfortunately they are only available privately.

I can offer them as part of my aesthetic practice, but not available on NHS. Tailored advice will be given in clinic.
